Awake craniotomy: principles, practice and outcomes - Kris Sivarajan, Pietro D’Urso and Catherine Furay

Talk synopis:

This session provides an overview of awake craniotomy from a neurosurgical standpoint, with a focus on the essential role of the anaesthetic team in ensuring patient safety, comfort, and optimal outcomes. Topics include indications, patient selection, intra-operative workflow, anaesthetic models (asleep-awake-asleep vs. MAC), and common challenges encountered during these procedures. Emphasis is placed on preoperative planning, multidisciplinary coordination, and real-time intraoperative collaboration.

Learning objectives:

By the end of this session, participants will be able to:

  • Understand the neurosurgical rationale and goals of awake craniotomy
  • Anticipate and manage common intraoperative challenges (e.g. seizures, airway compromise, agitation).
  • Collaborate effectively in multidisciplinary planning and intraoperative communication.

Complex spinal surgery - Glyn Smurthwaite, John Large and Matthew Lea

Talk synopsis:

Complex spinal surgery is clinically challenging and requires a truly multidisciplinary team approach throughout the perioperative process.  Many of our patients have complex spinal deformities which makes positioning for surgery difficult if not impossible using traditional equipment available.  We have developed a bespoke 3D positioning system in-house with our clinical engineering team to allow safe spinal surgery to be performed.  The presentation discusses the team based approach in dealing with the challenges associated with this group of patients.

  • The multidisciplinary team is not just doctors, nurses and healthcare staff
  • Communication and collaboration is key to success
  • Clinical engineering departments are a valuable group that can help with clinical problems to facilitate and improve safe patient care

Deep Brain Stimulation surgery

Synopsis:

This session provides an in-depth overview of the surgical and anaesthetic considerations for Deep Brain Stimulator (DBS) insertion. It covers the principles of functional neurosurgery, indications for DBS in movement disorders, and the multi-disciplinary approach to patient selection. The lecture details the surgical technique, anaesthetic management during DBS insertion, and the considerations for patients with existing DBS systems undergoing other surgical procedures. It also highlights recent and emerging technological advancements in DBS systems.

Learning Objectives:

  1. Understand the principles and indications of Deep Brain Stimulation (DBS) therapy in the management of movement disorders.
  2. Describe the anaesthetic considerations and perioperative management strategies for patients undergoing DBS insertion.
  3. Recognise the specific precautions and adaptations required when anaesthetising patients who have an existing DBS device.
