ADRT and ICU: reflections on a challenging case

Talk Synopsis:
Dr MacDonald will present a case of an advanced refusal of treatment in a patient who sustained life threatening traumatic injuries. We will consider the impact of the ADRT on the patient, family and treating clinicians and his reflections on what went well, what he would do differently and the overall experience.
Learning Objectives:
1. Examine the legal framework for the advanced directive for the refusal of treatment (ADRT) and how this relates to clinical practice alongside ethical principles.
2. Consider how the duties of a doctor relate to ADRTs including human and system factors.
3. Strategies that can be used to minimise blood loss and reduce impact of anaemia.

Shared Decision Making in High-Risk Anaesthesia and Surgery
Dr Tina Duff, Consultant in Anaesthesia and Intensive Care Medicine at Manchester Foundation Trust
Talk Synopsis:
Dr Duff will explore the different approaches to discussing risk with patients and how we can communicate this effectively. The importance of health literacy will be reviewed and how we can support patients in receiving and understanding health information. Informed consent needs to include information that is important to each individual patient and addresses what matters to them.
The importance of shared decision making in individualised patient care will be reviewed. The national expectations for shared decision making as a standard of care and how this can be delivered will form a key part of this talk. The need for a cultural shift to a team approach to shared decision making and how this can be incorporated early into the patient journey will also be considered.

Shared decision-making in anaesthesia – Serious Medical Treatment (SMT) cases; best interests decisions, and navigating legal aspects of withdrawal of life-sustaining treatment.
Learning Objectives:
1. A refresher on the Mental Capacity Act 2005 and best interests decision-making relevant to SMT cases.
2. Better knowledge of the Court of Protection and legal processes relevant to SMT cases.
3. Awareness of when to seek legal advice / when to consider approaching the Court for decisions in respect of patients who lack mental capacity.
